The Brookfield Publishing Co. Lineage Forms are in two Parts, Part I is to establish descent of a candidate from an immigrant ancestor. Part II is to establish descent of an immigrant ancestor from a qualifying ancestor for a particular Old World Hereditary Society. For candidates establishing descent from an immigrant ancestor with an established lineage, it may not be necessary to submit Part II. When the immigrant is either unknown or doesn
't have an established lineage, it will be necessary for a candidate to complete Part II.You will be able to access the Lineage Forms in two ways, the first is to click on the pages below and you will be able to view them.
Part I: Candidate to Immigrant Ancestor
Part II: Immigrant Ancestor to Qualifying Ancestor
The second way to to download them. Click on Part #1 or Part #2 to access to PDF files. To view these documents you need to use a PDF file viewer such as Acrobat Reader or Acrobat Exchange from Adobe Systems, Inc. Acrobat Reader is available at no charge from Adobe Systems world wide web site.
A third variation is a typewriter enabled version of Part#1 or Part#2.
The Forms indicate that copies of reference materials are to be included with the Forms in addition to the citation of the references. In addition, details of Immigration, including ship, port and other information and authorities for immigration are requested. As the information concerning the immigrant ancestors is included on the Web Site, the information concerning immigration will hopefully be included on the page for the particular ancestor.
